Job Description:
Crisis Preparation and Recovery is an innovative behavioral
health agency specializing in providing niche services to the
community. The SMI Evaluator is a position best suited for
experienced clinicians with a behavioral health background who
enjoy writing, diagnosing, interviewing and developing a
comprehensive and professional evaluation used to determine whether
someone meets the criteria for SMI (Seriously Mentally Ill)
services. The position allows for a flexible work schedule for
those who are self-starters with excellent time management skills.
While it does require a high level of independence, the candidate
will enjoy the support of management and team members. This is not
a remote position and requires local travel to area hospitals.

Duties:
- Leading a clinical interview with the goal of obtaining enough
information to make a DSM 5 clinical diagnosis, narrative on
impairment, and recommendation for/against an SMI
determination
- Use of both face to face and telehealth platforms to conduct
clinical interviews with individuals in a variety of settings, to
include, but not limited to, inpatient, residential, detox, and
outpatient
- Travel to client locations as needed
- Efforts to obtain and review collateral records and reports to
substantiate diagnoses and recommendations
- Email/telephonic/in person coordination with social workers,
family members, guardians
- Complete interim service plan and risk assessment on all
clients who participate in evaluation process
- Use of counseling skills to develop rapport, de-escalate acute
stress, and respond to emotional reactions that may surface during
evaluation process
- Submit all documentation within the required timelines
- Use of EMR for all documentation, coordination of care, and
case management
- Respond to and staff with Solari doctors as needed
- Respond to all internal and external emails and phone messages
by close of business on staff's scheduled work days
- Mandatory attendance at monthly Department Meetings, Individual
Supervisions, and required company trainings
- Compliance with required Relias trainings, and maintenance of
expirables
- Assist in training new staff
- Other duties as assigned
Qualifications and Education Requirements:
- Current, unrestricted license issued by the Arizona Board of
Behavioral Health Examiners: LCSW (Licensed Clinical Social
Worker), LMSW (Licensed Master of Social Worker), LPC (Licensed
Professional Counselor), LAC (Licensed Associate Counselor), Ph.D
(Doctor of Philosophy), Psy.D (Doctor of Psychology).
- Documented post-master's experience for two (2) years,
including experience within the past 24 months providing behavioral
health services, or an equivalent of experience, skills and
education. Appropriate reference checks by letter or telephone as
requested. Background check, as required by state.
Other Items:
- Provide evidence of professional liability insurance coverage
in the amount acceptable, minimum $1/$3 million.
- Must comply with training and supervision requirements during
first 90 days of employment, and throughout employment.
- Must be able to be paneled with various insurance companies
without restrictions.
- All licensed, certified, and registered staff will be monitored
regarding restrictions.
- Appropriate reference checks by letter and telephone as
requested.
- State-required background check.
- Medicare/Medicaid sanctions, criminal violations, subsequent
malpractice actions, and any settlement(s) made on behalf of
practitioner.
- Within seven (7) days of hire, apply for and/or present and
maintain a Class One Fingerprint Clearance card. If application is
necessary, a copy must be submitted for inclusion in personnel file
until card (and/or renewals) is received.
- All required renewals must be submitted 30 days prior to
expiration date. It is the duty of the licensee to keep all
renewals active and current and report any mandated changes to
Arizona Board of Behavioral Health Examiners immediately.
