Clinic-Based Telehealth Role for Experienced FNP This is an
ON-SITE position at our Banner Health Center plus clinic in
Glendale Join Banner Health‘s most innovative health campus in

seeking an experienced Family Nurse Practitioner (FNP) for our
state-of-the-art Banner Health Center Plus. This unique
clinic-based telehealth role combines the convenience of virtual
care delivery with the support of an on-site, collaborative team
environmentideal for providers passionate about innovative,
team-based primary care. Some Details About the Position:
Clinic-based on-site at Banner Health Center Plus, 7701 W. Aspera
Blvd., Glendale, AZ 85308 Provide telehealth visits to established
patients and manage clinical inbox for care coordination Flexible
scheduling: 4x10s or 5x8s shifts Medical Assistants, Front Office,
and leadership support Support of an integrated system including
specialists, urgent care, and more! About Banner Health Center
Plus: Our Glendale Banner Health Center Plus, located off the Loop
101 and 75th Avenue, offers primary care, pediatrics, specialty
medicine, imaging, lab, and pharmacy in one convenient location.
This state-of-the-art facility provides a collaborative, team-based

Requirements: DEA with prescriptive authority required FNP
designation with AZ license eligibility Minimum (1) year of
experience as an FNP in a primary care setting Patient-focused,
