AccentCare, a leader in home health care services, is seeking a compassionate and professional Hospice Clinical Liaison RN for inpatient care over weekends. This role is pivotal in bridging the gap between inpatient care and hospice services, ensuring a seamless transition for patients and their families. The ideal candidate will be responsible for evaluating patients' needs, coordinating with healthcare teams, and facilitating the implementation of hospice care plans. By joining our dedicated team, you will be instrumental in enhancing patient care and support during critical times. Assess patient eligibility and coordinate admissions for hospice care from inpatient settings. Communicate effectively with patients, families, and caregivers about the hospice care process and expectations. Collaborate with healthcare providers to gather patient information and medical history to support a seamless transition to hospice care. Educate clinical and non-clinical staff about hospice services and patient care protocols. Participate in case conferences and contribute to patient care planning and documentation. Ensure compliance with healthcare regulations and hospice standards. Strong clinical skills in hospice and palliative care environments. Excellent communication and interpersonal skills. Comprehensive knowledge of healthcare regulations related to hospice care. Ability to work independently and manage time effectively. Proficient in using healthcare information systems. Medical Insurance Dental Insurance Vision Insurance 401k Valid RN license in the state of employment. Certification in Hospice and Palliative Nursing (CHPN) preferred.
