Presbyterian is hiring a Registered Nurse Care Manager who
pro-actively manages the care for patients along a continuum by
facilitating collaborative care between patient, family/support
system, physicians, and other ancillary departments/members of the

Opportunity: Full time FTE: 1.00 Job Exempt: No Work Shift: Days
(United States of America) Responsibilities: Assessment of an
identified patient population based on symptoms and medical
history. Development of a plan of care based on preventative care
needs, using standard based practice guidelines. Collaborates with
patients to update problem list, current medication list and
coordinates visit plan. Serves as patient advocate. Serves as
communication link between Inpatient Care Manager, HHC Care Manager
and PHP Case Manager. Documentation within scope of practice. Other
duties as required Qualifications: Associate’s degree in nursing
State of New Mexico or Compact State Nursing License BLS
certification REQUIRED at at time or hire 1-3 years ambulatory
