Job Description Job Description Nurse Practitioner (NP) / Physician Assistant (PA) - Post-Acute, East Valley, AZ Location: Mesa, AZ (Primary) | Some coverage in Chandler Setting: Skilled Nursing & Assisted Living Facilities Schedule: Full-time, weekdays on-site 9 AM – 5 PM , plus weekend rotation once or twice per month Employment Type: 1099 Independent Contractor Compensation: Productivity-based pay with potential to earn up to $300,000 annually About the Opportunity We’re hiring a full-time Nurse Practitioner or Physician Assistant to provide ongoing care within post-acute and long-term care settings across Mesa and Chandler . This position offers steady weekday hours, autonomy, and a manageable patient load in a supportive clinical environment. Providers typically see 15–16 patients per day in Skilled Nursing and Assisted Living communities, focusing on established, stable residents and new admissions as needed.
