Overview Plans, organizes, and assists in the provision of direct and indirect resident care following established policies and procedures. Orients, instructs, and supervises personnel. Perform duties of an LPN as required within Arizona State Board of Nursing Scope of Practice. The LPN is also responsible for his/her own actions. Assumes responsibility for his/her continued self-development in order to help plan and provide quality resident care. Responsibilities Receives report and gives report to C.N.A.'s Makes rounds of assigned neighborhood at beginning and end of shift and sees each resident at least once per shift. Prepares and administers medications and treatments within the standards set forth by the Arizona State Board of Nursing and Friendship Village Health Care Policy. Performs blood glucose monitor and administers insulin according to physician orders. Give O2 therapy per physician order. Records effects and reactions of such medications and treatments. LPNs may administer peripheral IV medication if IV certified and according to policy. Functions as a Team Leader and assigns duties for personnel on his/her neighborhood including positioning, ambulating, and exercises for optimum comfort of residents and prevention of skin breakdown and contractures. Orients, instructs, and supervises new employees in the department. Evaluates and assists in developing a Nursing Care Plan for each resident. Implements, reviews, revise, and update each individual care plan on an on-going basis. Encourages interest and participation in facility activities programs. Assists with transportation of resident to/from activities. Assists with rehabilitation of residents according to the medical and Nursing Care Plans. Consults with supervisor, peers, and physician in planning and providing continuity of individual care. Abides by established policies and procedures for Health Center and assures compliances with these by nursing personnel. Assists with admissions, discharges, and transfers of residents including completing required forms for transfer to another facility. Evaluates residents upon admission, and follows through on processing all admission orders. Documents condition status on all Medicare residents per shift. Makes independent decisions regarding nursing care. Observes, reports, and records all discernable symptoms, reactions, and changes in conditions of residents. Maintains resident records accurately and completely. Maintains nutrition and hydration Hangs and monitors nasogastric/G-tube feedings, i.e. Ensure, Isocal, etc. Instills oral medications per physicians order via NG and G-tube. May irrigate, instill medications, in G-tube per physicians order, and as needed. May give bolus nutritional feedings via syringe through NG or G-tubes per physicians order. Oral intake Assures monthly weights are recorded accurately and if there is a 5 pound or more weight change, that dietary and physician are informed. Assist with transporting resident to and from the dining area. Is responsible for serving or supervising the serving of nourishments. Sees all residents have fresh water. Records all intake and output as ordered. Evaluates emergencies and determines emergency measures to be used. Immediately reports to the nursing supervisor conditions which require the professional skill of a supervisor or physician. Maintains elimination Gives enemas (Tap water, SS, oil retention, fleets). Irrigates ostomy and changes colostomy/ileostomy dressings as needed. Inserts and removes urinary bladder catheter. Assures that urine specimens - sterile, midstream, and clean, and stool specimens are collected and sent to the laboratory. Assures that all residents have had a bowel movement within the last 9 shifts. Completes, evaluates, and signs all Incident Reports on assigned neighborhood and completes the Post Fall Investigation with the supervisor. Receives, notes, and records physician orders. Responsible for completion of daily assignment. Qualifications Graduate of approved school of nursing. Minimum 2 years recent clinical experience in caring for older adults in acute and long term care settings. Must possess the ability to organize, plan, delegate and supervise. Must possess excellent time management skills. Effectively read, write, and communicate in English. Current LPN license in the state of Arizona or compact state. Health Care Provider CPR, required.
