We focus on Integrity, Compassion, Relationships, Innovation and Performance as we empower people to achieve better health and well - being.As a Field Based Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education.This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8am - 5pm. It may be necessary, given the business need, to work occasional overtime.Primary Responsibilities:

  • Create a positive experience and relationship with the member
  • Keep the member out of the hospital by supporting regular visits to their primary physician
  • Keep member actively engaged with their primary physician
  • Support the member to ensure pick - up of their Rx
  • Proactively engage the member to manage their care
  • Provide member education
  • Support transitions of care
  • Help to keep members compliant with their care plans
  • Partner with care team (community, providers, internal staff)
  • Knowledge and continued learning of community cultures and values

    • High School Diploma / GED (or higher)
    • 2+ years of Care Management Experience
    • Resided within the local community for 2+ years
    • Knowledge of culture and values in the local community
    • Familiarity with the resources available within the community
    • Experience with outbound calls to members, Physician's, Pharmacy, Providers or similar medical facility
    • Knowledge of disease management and treatment
    • Proficiency with Microsoft Word, Excel and Outlook (ability to create, copy, edit, save and send documents, spreadsheets and correspondence)
    • Ability to navigate a PC to open applications, send emails, and conduct data entry
    • Ability to be in the field 60% and home based 40%
    • Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area
    • Ability to work Monday-Friday; 8am-5pm Arizona Time
    • Reside within Maricopa County (Phoenix, AZ, Tempe, AZ; Mesa, AZ; Scottsdale, AZ; Paradise Valley, AZ )
    • Dedicated work area established that is separated from other living areas and provides information privacy
    • Ability to keep all company sensitive documents secure (if applicable)
    • Live in a location that can receive a UnitedHealth Group approved high-speed internet connection or leverage an existing high-speed internet service
    • Full COVID-19 vaccination is an essential requirement of this role. Candidates located in states that mandate COVID-19 booster doses must also comply with those state requirements.       • Bachelor's Degree (or higher) in social work and / or healthcare administration -
      • Field Based experience
      • Community Health Worker (CHW) Accreditation
      • Experience working in Managed Care, Behavioral Health, Substance Abuse and Medical Care
      • Knowledge of AZ Medicaid & Medicare population
      • Bilingual fluency in English and SpanishSoft Skills:
        • Excellent communication skills
        • Ability to work autonomously and ability to self-direct
        • Ability to remain focused and productive each day though tasks may be repetitive
        • Ability to work in multiple systems and applications to complete daily tasksPhysical and Work Environment:
